Dalteparin

Dalteparin is a low-molecular-weight heparin (LMWH) that acts as an anticoagulant. It helps prevent the formation of harmful blood clots by enhancing the inhibition of coagulation factors Xa and IIa by antithrombin. It is administered via subcutaneous injection and has a more predictable pharmacological profile compared to unfractionated heparin.

Dosage by Body Weight

Dosage must be individualized based on patient weight, indication, and renal function. Typically administered subcutaneously once or twice daily. Anti-Xa level monitoring may be required in pediatric patients, pregnant women, or those with severe renal impairment.
